WELCOME to the Elk Avenue Medical Center. We started this practice in order to provide more medical care choices for the residents and visitors of the Crested Butte area. Our goal is give our patients the best possible health care in a timely and compassionate manner. We continually strive to maintain the most state-of-theart medical practice in Gunnison County.

MISSION: To ensure the highest quality healthcare for our patients in an efficient, timely, friendly and professional manner.

HISTORY: The Elk Avenue Medical Center was established December 2002 by Dr. Lee Lynch, combining clinicians from various fields and backgrounds.

STAFF: Our main clinicians, with a wide range of professional experience, include Dr. Lee Lynch, Colleen O’Sullivan PA-C, Dr. Pam vonMatthiessen, and Dr. Gloria Beim. They are all Board Certified in their respective fields. Our support staff includes Registered Nurses, Paramedics, EMTs, and experienced administrative staff. From the time you make an appointment to the conclusion of your treatment, the staff you meet will be friendly, proficient and as concerned about your health as you are. Your privacy is important to us -- we are HIPAA compliant.

AVAILABILITY: We offer extended hours, opening early and staying late. We are open six days a week year round, and on Sundays during the winter and summer. You can schedule ahead with the provider of your choice or be a “walk-in” anytime. There is always someone available who is ACLS certified. Also, for your convenience, we can draw blood in our office with written orders from any clinician. We then send the blood to Gunnison Valley Hospital for processing. In addition, our prescription services department offers the same extended hours as our clinic.

PRESCRIPTION SERVICES: We stock the largest inventory of prescription medications in Crested Butte and dispense them directly to our patients, eliminating a 30 mile trip to the public pharmacy in Gunnison. The medications we stock are only available for prescriptions we write, or refills we have authorized. We can process your prescriptions through many insurance companies, so in most cases, you pay only the copay. Please call at least one day ahead for refill requests.

PAYMENT/INSURANCE: Payment is expected at the time of service. We do accept co-payment for those insurance companies we contract with, including Blue Cross Blue Shield/Anthem, CHP+, Rocky Mountain Health Plans, MMA, Great West, Sloan’s Lake, Aetna, Tricare, Medicare, and Medicaid of Colorado. We do not bill any HMO’s, except those associated with Blue Cross Blue Shield. You are welcome to call our office for more information.

COMMUNITY OUTREACH: 5 Minutes for 5 Bucks: Every year we hold at least one fundraiser called “5 for 5”. This is an opportunity for anyone to have a 5 minute medical consultation with a clinician and pay only 5 dollars. The money raised goes entirely to a local charity.

9 Health Fair Lab Review: Our clinicians provide free 9 Health Fair Lab reviews. If your results indicate action, we will suggest that you schedule an appointment.
